Kalamazoo Community Foundation Scholarship Forms

Many Kalamazoo Community Foundation scholarships require you to submit financial information, one or more recommendations, and/or other information in addition to the online scholarship application and your official transcript. Carefully review the eligibility criteria and application requirements for every scholarship for which you are applying to see what additional information or forms may be required.

  This two-page form must be completed and uploaded to eGRANT when prompted.

• Brady & Patrick Gallagher Memorial Scholarship Teacher Recommendation Form (DOC | 83KB)
  If you are applying for the Brady & Patrick Memorial Scholarship, this form must be completed by an arts teacher and mailed to the Kalamazoo Community Foundation.

• Mildred McConkey Memorial Scholarship English Coursework Form (DOC | 62KB)
  If you are applying for the Mildred McConkey Scholarship, this form must be completed and uploaded to eGRANT when prompted.
• S. Rudolph Light Medical Education Scholarship Dean’s Form (DOC | 48KB)
  If you are applying for the S. Rudolph Light Medical Education Scholarship, this form must be completed by a representative of the dean’s office of the school you attend and mailed to the Kalamazoo Community Foundation.
• Clarence L. Remynse Scholarship Recommendation Form (PDF | 136KB)
  Use this recommendation form only if you are applying for the Clarence L. Remynse Scholarship.
